Most will say that the art declined (which is true), but I think that Fujimoto is also intentionally drawing Denji in a way that makes him look unappealing compared to early P2.
It's not like he's drawing Yoru, or Fami, or Yoshida, etc. with huge eyebags or weird faces.
The two characters that really keep looking creepier by the minute are Denji and Death.
