They remade every prerendered background in 3D.
They added voice acting.
They reworked the combat system.
They added new demons.
And then there's the rest of technical stuff like bumping the resolution, texture quality and framerate (which are the 3 aspects that actual low effort remasters tend to focus on not caring about anything else).
