For Isaac Afterbirth and Afterbirth+ are kind of awkward points in the DLC progression. They kinda fuck with the balance of the base game that isn't fixed until Repentance and add Greed mode which a lot of people dislike.
If you've played a lot of the game then I'd get all of the DLC, otherwise I wouldn't get just Afterbirth.
