They can be anywhere on the spectrum, but generally any alterations they accept are going to be enhancements, maybe with a drawback like mental instability but who cares they're already actively fucking with Chaos. For plague marines I wouldn't say they can move like lightning especially not for an Astartes, but they don't care because why dodge or be concerned with hitting first when you can just eat the hit and when your own punch lands it's going to wreck. The setting fully supports CSM being as traditional to their old ways as you like, or not, you have a lot of freedom of choice. Honestly one of my favorite things is members of CSM warbands being veterans of the Great Crusade, still seething about it and still kicking ass.
