How about
>every obstacle, creature and item is modelled on a voxel grid
>ranged projectiles are modelled as lines starting from the end of the attacker's weapon
>bullets are straight lines, thrown objects and arrows make ballistic arcs
>they have a random angle error depending on attacker's skills and weapon properties
>the projectile hits wherever the line intersects a voxel, damaging that object and triggering any secondary effects (e.g. explosions)
>also if you try to make a ranged attack with an enemy adjacent to you, you need to make a CQC check against that enemy, and if you fail you shoot in a random direction and not where you were trying to aim
