I would imagine there's a general threshold of sensations that their models can endure before showing "physical" effects on them, the whole pain of it comes from their minds who instinctually conclude that certain sensations must cause the equivalent in the bodies they have. Kind of like a placebo thing but instead of feeling better through mental gymnastics is about feeling the whole arrange of stimuli of your existence in a world where that concept is meaningless
