She's really not. The clothes are a technical limitation because there's no factionless medical outfit in the game and you're never meant to fully see real Iosefka through the door. By the time the game starts the Healing Church and all its members are being hunted down which is why they've retreated to Cathedral Ward, locked all the doors to the district and you can find a myriad of Church corpses all over the streets, slain by Yharnamites. For Iosefka to operate a makeshift hospital (given it looks like a regular manor, has no rooms dedicated to medical care and the operating tables are haphazardly shoved into what seem like normal living spaces) open to the public, refining her own blood and not being hunted by the Yharnamites patrolling right outside her door it shows that she's not part of the besieged and hated Church

Who himself, like the cave under the clinic, is a whole can of worms because there's a reason Iosefka doesn't appear in the intro, he disappears after it, at no point is he referenced as an employer of hers and she doesn't bring him up once: 99% of his character was cut
