everyone does not need the dlc's. You can turn off the dlc's that you own and play with your friends who don't own the dlc, in a save that has the dlc turned off for everyone. The only thing you can't do is have people that don't own the dlc playing in a lobby that has the dlc turned on. It'll prevent the game from starting until everyone turns it off or everyone buys it.
The one exception to this is the preorder and special edition dlc's, those are active for the host if the host has them and the other people don't need to own those, since it would be a hassle since not all of those are available. So this all only applies to the actual void shadow and lex imperi dlc.

Everyone in the lobby does everything the captain does. So yes, they'll watch all your romance dialogue.
In my coop games my friend who had never played before was the captain and he romanced Cassia.
