The problem is that mandatory residency training has just become another fail upwards scheme.

By the time you complete residency, you should be able competent to identify, workup and manage all the common conditions.
The residents at all the major hospital system programs are instead relegated to being EMR data entry slaves, who need to ask the fellow, another higher level trainee, what to do. The fellow is the only one with agency to practice and perfect skills, and the only one to get direct feedback from the attendings.
This problem is exacerbated in all academic medical institutions with fellowships because the residents are coached from the start of residency to strive to specialize and become "something"ologists, and stay on track to preferentially practice the precursor material for that specialty. then they pretty much mail it in on all other aspects, and consequently they suck at general practice.
