The grotesque has been a part of art for as long as art as been around having deliberately grotesque protagonists has been around just as long.
The thing that defines wokeness is not presenting ugliness, but normalizing it. Characters are obese, women are deliberately made unappealing, despite any real eroticism at all there is lots of pro-gay signaling, etc. Wokeness is almost daring you to notice the ugliness so you can be socially punished for it.
