I wouldn't call it a flawed aircraft, as the other anon said it's half a chopper, and choppers have a nasty habit of dropping straight out of the sky. Frankly, it isn't even that uncommon for planes. I think the V-22s problems are threefold: 1) it can do more than a plane or a helicopter can, so it gets pushed a little too hard by the occasional pilot or CO and it crashes, 2) relatedly, because it is both helicopter and plane, it is at risk of both the kinds of physical conditions that cause planes to crash (for example, a compressor stall), and the kinds of conditions that cause helicopters to crash (for example, vortex ring state), and 3) the Marines famously fight for scraps in the budget, and that plus the oorah tempo-tempo-tempo jarhead culture probably mean they don't get nearly the amount of servicing they should, meaning shit wears out, breaks, and the bird crashes.

I suspect there's room to improve, and it's noteworthy that Bell's new MV-75 keeps the engines fixed and only tilts the rotors and the dirveshafts, since allegedly that will be the new Blackhawk replacement and Bell also built the V-22. But all-in-all, I think the V-22 gets a harsher reputation than it deserves.
