Trannyshit has somehow lead to a nightmare fuel scenario even worse than ORAS. Tranny Sapphire would snap out of the delusion at 25 and "detransition" and try returning to normal life as a deformed girl, but if Ruby had gone tranny too he'd probably just end up sewer sliding around the same age or later.
