its because Radahn fits the role of the Crimson King/male and Miquella fits the role of the White Queen for the Alchemic Rebis
like Marika before him, MIquella is trying to finish the Great Work to be come a true god, for whatever reason, Marika's attempts to create the Rebis with Radagon was not perfect, they were not in opposite but harmony.
Mind you, the actual gender of the beings in question is not relevent, simply that they fit the "nature" of the White Queen and Red King, and all the natures that come with each.
the entire DLC is about Miquella performing the many steps to create the Rebis, there's even a direct and obvious tell with the Putresence being a substance/entity.
