My first playthrough also ended with frenzied flame but purely on accident
Melina was already dead by then and I went for the big door not thinking it would immediately start a cutscene that locks me into the ending
Didn't know about the needle so felt really guilty for the rest of the run. Probably shouldn't have, hands down the best ending cutscene in the game

I stumbled into the frenzy flame ending completely on accident on my first playthough (didn't expect the huge doors to start a no-going-back cutscene lmao) and have since thought it's the most interesting one, what with the questline itself, the revelations it brings, and the kino cutscenes at the end.
