I like to imagine that Mohg's crew was much less deranged prior to Miquella entering the picture. Just look at the contrast between Ansbach and Varre. I think at first Mohg and his nobles and pureblood knights were only concerned with cultivating their powers, and only after the cocoon was made did they become near-mindless killers collecting blood.
