Time Spy results show your video card is working just fine, it's harder to get an average score on 3DMark because of self-selection bias (people who care about PC performance tuning and OCing are more likely to run these tests, skewing the averages).
I don't have a solution for that Radeon glitch you're describing, try switching the display cables and connecting it to the DisplayPort instead of HDMI.
