it depends, sometimes I want to jack off to something but there's nothing that scratches the itch for me on rule34 that I can find so I make it myself.
other times I'll want to do something with a specific character but not sure what so I just mess around aimlessly in blender until I think of something and sometimes I'll have an exact scenario+pose in mind ahead of time
