Yeah, it's basically non-existent in the original, the author put some of part 2 stuff as you probably noticed, and expanded the gladiator guilds subplot with a new, massive questline. The arena itself got a rework, too. Overall the western part won big time in the update. Lots of new building, lots of the dhorn presence, and much more verticality as mentioned by another anon. v1.0 west is not really memorable.
But yeah, nu-sewers is straight up the hardest part of the game I'd say, since you're forced to survive out there pretty early and with barely equipment, shitty crawling skills, and without a single idea where to go since the in-game map is not informative in the slightest. The fact that it's split in ~3 bigass maps doesn't help either.
