If I remember correctly, these games run on Direct3D9, which is not really good at loads of polygons. NepU has a low amount of polygons by modern standards, but quite a lot of polygons by DirectX9 standards. Another good example is Grand Theft Auto IV, one of the worst-optimized PC ports of all time.
Try using DXVK or dgVoodoo with the game, see if that smooths out the frames. You may need to tweak the configs to get optimum performance though.
