Yeah sometimes leaders get killed but the ones at the top are usually well-protected which probably makes them feel a bit invincible. As soon as they feel vulnerable they will just run away and hide. Look at Assad. He was perfectly happy to kill Syrian civilians with sarin (a chemical weapon), while he felt he was well-protected. But as soon as Assad felt vulnerable, when others tried to topple his government, he fled to Russia.
