Making an eroge is easy. The only requirement for it is to have sex scenes. Problem is the amount of games, mostly straight, that have sex after losing whatever gameplay mechanic the game is based around. Thus optimal gameplay means avoiding sex and like, what the fuck do you think we are here for? Then there's an amount of devs (and players) that insist that characters having sex with literally anyone besides the main character on any context counts as NTR.
Devs that feature sex as reward for good gameplay are one step above, but still far beyond the ones that make sex directly part of the gameplay. Preferably if it isn't just "break free from sex" (I'll never forgive Monster Girl Quest for this, and a few other things)
Let's brainstorm a little, how do you imagine (non-lethal) sexual combat between men? If it's turn-based (mostly to make it mechanically complex but still giving the player room to think), I would say something like different targetable body parts (like Fallout to give an example) that react different when touched, the ability to show off certain parts to mesmerize the opponent, breaths to avoid cumming, clench the ass with various intents. How does cumming (and in what location) affect if one or the other is the winner? Cumming handsfree is different from getting a handjob is different from doing it inside an ass and one or the other could be the dominant one that made it this way.
