Not really? Nothing wrong with just reusing existing monsters and assets. Total Conversion doesn't mean make entirely new thing out of nothing. It means reusing the game's assets to make something different out of them. For the easiest example, take the general landscape of Act 1's grove/goblin area and build a new fort with a forest around it. There you got a "new map". Now add some NPCs and quests and you got yourself a total conversion of BG3 for a new module that you can do whatever you want with. It's not always as ambitious as it sounds.
But yes, a game with so much detail, animations, dialogue and skill systems obviously will take effort to do anything with. But you must underestimate the sheer autism of CRPG modders.

I dunno. Divinity OS2 had some. Fucking Skyrim has a million. As I recently learned Cybperunk has been completely broken to just do whatever the modders want at this point... It's really not that unfeasible. Especially because as I said, Larian literally gave modders all the tools they need freely.
Yes, if you want high quality campaigns, it will take a lot of effort. But smaller projects like 99% of BG1&2 modules were, is totally doable.
