There is definitely a lot of inconsistency which is largely down to perspective. Panels drawn from below or above or from the side, etc. It's the same reason characters head sizes change a lot in proportion to their bodies. The admirals are often depicted with tiny heads when the view is taken from beneath them.
The only full panel I could find that had Kaido in shot with another character (zoro) is pic related.
