As Ansbach says, Miquella used Mohg to enter the Shadow Lands. I think we can infer that it is specifically Mohg's blood that has some connection to the realm. This would explain how and why we need to touch the withered arm - Miquella's corpse is filled with Mohg's blood, and gathered blood from Bloody Fingers as the Lords of Blood Exultation implies.

I know generally how many years the lore and story span is controversial but I would like to think it's been a couple centuries since the Shattering. However Miquella only seems to have made his move in the shadow realm shortly before we arrive, as he breaks his great rune while we are there. Was he chilling for a long time either with Mohg or with Messmer?
Yeah its difficult to parse. Especially when you factor in Miquella likely found Torrent, a horned horse, in the Scadulands (Or it was handed to him by Radagon/Marika? But this wouldn't refute that point, since Marika is from the Scadulands)
One odd thing to keep in mind is Radahn's revival appears to have taken place just before Miquella entered the gate. "Lord brother, at last you are returned".
