He's focusing on player agency in perpetrating the torture, rather than the tone with which the torture is handled, and (perhaps intentionally) missing the fact that everything that happens in a video game ultimately exists for the player's enjoyment.
Degrees of Lewdity is the first counterexample that came to mind. It's clearly and unequivocally a porn game, but the player character is almost always in a passive role. You're not fucking, you're getting fucked, at least supposedly against your will. Even in consensual sex encounters, you have very little control.
But obviously, even though your character is getting traumatized by events outside her control, you the player are choosing to play this of your own free will. The game was made so you could watch a child get raped, and its world is built to facilitate the rape.

So how is the design behind Scav Game any different? The game developers deliberately made the game generate a chasm filled with traps and unstable footing, so your character can get hurt, and much effort was clearly spent on conveying the this suffering to the player. Anon has repeatedly mentioned how well the game accomplishes this, using audio and visual cues to show the pain rather than merely describing it.
I haven't played the game, so I don't know if it really fetishizes the pain as much as Anon makes it seem, but Anon's descriptions are similar to what someone would write for a porn game they enjoy. It's clearly porn for him, even if that wouldn't be the case for everyone.
