It did happen before because a lot of quest hubs were missing. Thorium Point, Revantusk Village (patch 1.5), Steamwheedle Port, Silithus (1.8). I think LHC also lacked stuff before Naxx patch. Plus various other quests that were added later. Class quests for example were added in 1.7-1.8.
