NTA but Doctor had always been like that, even in Babel. The reason he decided to kill Theresa was because he saw it as a mercy to the Terran people (see: "pussied pacifist") and a way to end the war faster. Kal'tsit also knew that he might make that decision, which is why she regrets waking him up after seeing Theres's corpse. When we saw Doctor from other characters' POVs, that's when he comes off as merciless. As far back as Vigilo we see that's not the case, and in Operational Intelligence - The Anonymous Ones' War, Scout outwardly acknowledges that becoming Babel's tactician is making him more cold and calculating. We even see this in Babel, where Doctor begins to shut off his own emotions to get the job done without breaking down (something Closure notes in Vigilo when she says "the Doctor is becoming stranger and stranger"). Hell, the Doctor even says he "gets nervous around crowds" in Babel (BB-ST-3, iirc). If you read through the flashback segments with him talking to Kal'tsit, he seems like a chill father figure, something that's reinforced in Chapter 15. So...Doctor has always been more of a pacifist.
