The Sands of Fate trilogy of modules are the best epic level modules I've played. They take the player from 25 to ~40

The Lord of Terror, a diablo 1 remake, is another great hack and slash module. Starts to drag towards the end if you're going for 100% completion but pretty fun. I haven't played actual diablo 1 but I hear it's pretty faithful to the original
