It was made by and for fanfiction writers and readers. The reason why a tag like "Character A's small dick" redirects to "Character A has a small dick" and not "small dick" is because which character has the small dick is vastly more important for fanfiction then original works. A fanfiction reader will have preconceived notions and preferences of which character should have a small dick and it being the wrong one is a deal breaker.

This is also why it's highly unlikely that AO3 would implement anything more robust then the Original Work + Exclude Crossovers search for finding non-fanfiction work.
AO3 is open source https://github.com/otwcode/otwarchive and there are set up instructions made by someone else who hosts a version of it https://squidgeworld.org/works/34491/chapters/76104 so it is possible to spin up your own version better suited for original works if you really wanted to.
