Does it have to be a Western game? If so I nominate this game, Hades, as a SoL set in the underworld and every episode ends with the MC Zagreus dying.

I wouldn't want it to be accurate to the game just let whatever Japanese studio go nuts with is. Beach episode with all the Erinyes with an ocean of lava? Sure why not. An episode of Dusa get chased around by Cerberus? Go ahead.
