I generally recommend against the QMTech one if you have other options, since unlike the DE-10 and Retro Remake boards the QM DE-10 has a different pinout for the expansion boards and is not compatible with many standard accessories that would work fine on he Terasisc/RetroRemake DE-10 boards.

The RTC is just an RTC-add on that plugs into an internal port, it has nothing to do with SNAC.
It also comes with an N64 SNAC adapter.
The SNAC Adapter and RTC are just two separate extras, they have nothing to do with each other. If you already have a MiSTer and you want an RTC you can just buy an RTC add-on for it, they're pretty cheap. It's just so the MiSTer can keep track of the time if no internet connection is available (as well as instantly have the time on boot instead of waiting to get an internet connection), useful to keep track of save files and used in some cores (such as GBA and AO486) that support it.
