Obviously you rework the content too. No more stupid dogshit body checks, no more simon says DDR crap, give all fights and encounters enough leeway to get back on track after a raise or two, allow people to make mistakes and recover from them, it should be a struggle, not a carefully rehearsed dance where one misstep spells doom. Bosses should also utilize their attacks in a far more randomized fashion and be much less predictable, but they should either take longer to execute, or be more multifaceted, where getting hit by one half could be fine, but not by the second half, plenty of encounters have that with "element vulnerability" mechanic, but it is only used in service of forcing people to dance more.

It doesn't. It's impossible to truly challenge those that want to beat it, they will beat it anyway, so above else content should be satisfying first. I don't mean trashing the difficulty completely, things like Ultimates still should demand you bring your A-game, but the current state of FFXIV PvE has to go.
