The blocks in an outboard stand upright, so you'd either have to use a 90° gearbox to get the output shaft in the right direction, or completely rework the oiling system to lay the engine flat. The raw water cooling system dumps into the exhaust, so you'd have to build new manifolds and seal off the cooling system to run it with a radiator. Likely would run an electric water pump because you wouldn't have the impeller anymore since those are usually down in the drive unit. Plus a whole host of other shit that would come with flipping the motor sideways. It's a lot more fabrication work than is typically worth it, unless the boat engine is some oddball engine you couldn't otherwise get like that guy that threw a 2 stroke Evenrude V8 in his volvo. Or you could just hook the prop shaft of an outboard up to your trans input and leave the whole thing sticking out of the hood kek. It's way easier to marinize a car engine than de-marinizing a boat engine.
