If a diagram is showing a zeroed optic aiming for a target to the side of the gun, with the distance scaled down for demonstration purposes, making the arc more noticeable, then the entire gun should be angled somewhat upward to match the muzzle to the arc of the bullet's path, and the horizontal line coming out of the upward angled optic is what should be allowed to be visibly crooked in the diagram, because the zeroing adjustment angle is what's being exaggerated.

In reality, extreme angled zeroing involves special mounts that do angle the body of the optic to be visibly not parallel with the bore, since the adjustment range is limited, but the diagrams are usually illustrating the concept for basic sights that don't need that, not depicting mile+ rifle setups or grenade launchers.
