I assume he's wrapped up in the cinematic because he was still 'sleeping' in storage. You learn 621 is fucked up in the game too but the amount of medical shit stuffed in him like a pin cushion and the dialog really drive home that he's little more than the meat component of a machine. It just makes kindness of the disembodied waifu voice more powerful to think about if he's so fractured and barely human.
