I've been pondering why direct death seems to be the common ground where the world stops
Several forms of entertainment bask in the glory of physical or psychological torture (often for a prize that may not even be worth it), but somehow, a RL death game has yet to be made?
Overpopulation and the idea of leeches killing society? Why not popularize euthanasia? The western world has long since given up on theology at large. Afraid of not finding volunteers? As far as i'm aware slaughterhouse & casinos are still in business, and both of these have much more graphic destruction of one's life than a mere needle in the arm.
I do not believe there is this collective consciousness that this would be a slippery slope we couldn't recover from. History happens twice upon a time. Watching people kill each other used to be a form of entertainment.
