NTA but that's because they can't work with replayability. Most people only play through a game once. Look at the achievements for action games and you'll see the "clear the game once" achievement is probably 20-30% and the "clear the game twice" achievement is like 5%.
A game with a 10 hour playthrough time that you're meant to play through 3 times is considered less content than a game with a single, 30 hour playthrough.
A big problem is that the games are designed for multiple playthroughs with how normal mode is meant to be a tutorial for the combat systems and enemies and then on the higher difficulties it tests you. What action games probably need to do is find some way to drive normalfag players to beat the game on New Game+, but a crapload of people never play anything after credits roll.
Pic related, Assault Spy is a game almost exclusively played by action game fans but even then the achievements show that of the people who beat the Asaru campaign, only two of them actually played the entire second campaign with a different character, a different story content and extra stages at the end. Of course, even fewer beat Hard and Extreme modes.
