Yes, that happens. It's explained there's ruins and old cities that have them everywhere because of this. But again, it requires a set amount of negative energy. A single dead body or a couple separated by great distances won't count. I doubt a skeletal arm in the dirt counts the same as a dead body too, but maybe a bunch of skeleton parts from different bodies in a pile would be worse than a single dead body. We don't know if bodies ever stop.

Coyotes and wolves are relatively weak and we've done our damnedest to stamp them out of areas where we raise livestock, so I'd think we'd do the same and not have a skeleton spawner in our backyards, even if its "not a problem" (because it very clearly IS a problem waiting to happen, as evidenced by people knowing that liches spawn eventually).
We know that it only ever happens if an outside force like a stupidly strong unnatural undead appears and intentionally causes it.

The graveyard that got overrun by undead was within the kingdom and only happened because because he was a necromancer class. We see some characters in smaller towns burying their undead, and we know they don't have guards, so spawning undead requires a lot of bodies. The rate of them spawning requires a lot of negative energy, etc. Burning the bodies themselves at such a high rate might cause other problems, since it's not just the fact that dead bodies exist that creates negative energy, it's the negative aspect of it that does.
