That drawing does look more human, but it still 100% looks like a monster - a human-type monster, like a zombie or some kind of human-troll crossbreed or something. But in the game, the dome of his skull often looks like a dinosaur's bone helmet with a giant nose guard on the front, and his hair looks like tentacles. I know his backstory now but to me he still looks more like a half-melted zombie than anything else. (Which, as I said before, is pretty cool. I was disappointed to learn he was just old.)
