The Mafia was very powerful as well. They just didn't typically display their victims to the public like the Cartel does.

Mafiosos would've probably killed more of each other if they didn't found The Commission to make sure they weren't stepping on eachothers toes. The Cartels are too violent and too non-white to cooperate for their own benefit so they just end up killing thousands in turf wars, something the Mafia stopped doing by the 1930s.
