First Blood has to be done outside of dark urge duel because the cultists don't chant if you 1v1 her. For the Ceremorphosis one, defeating the Elder Brain I think implies it needs to die as opposed to being controlled. I assume you turned yourself into a squid, or did you have Karl or Orpheus become the squid? There's also a thing that enables achievements for modded runs.
