From the ground up they were throwing their shit at the wall hoping it stuck without actually knowing any long-term plans, and immediately cowering out when reception was negative. Filling out thoroughly who the Forerunner were, the retarded ideas behind the origins of the Flood and why the Forerunner apeified Humanity, most new characters being boring or absolute dogshit besides Lasky, old characters being killed off for shock value or being brought back arbitrarily and then disappearing again, most of the major-to-know details happening off-screen in books, comics and other media, and so forth. And when you did have something to play through, it would be padded out, buggy, and dumbed down for some dumb gimmick or really horrible decisions made.
Like, short of actually playing the campaigns and getting basic functioning fights, every single step of the process was fundamentally misunderstood and fucked from the ground-up to the top. 343 wouldn't have been able to even operate an original IP for how little consistency and how much mismanagement they had, never mind Halo.
