Ruka's illness isn't arrhythmia, and symptoms like that don't actually exist, so if he was too precise about that, he'd be heavily criticized. If Miyajima had accurate knowledge about that illness, she should have been cured by now.

I've heard that many times, but it wouldn't end with a meaningful frame in the Ruka birthday date episode without anything happening. I've been wondering about it ever since then, and if she's fully recovered, there's no need for Ruka to measure it now.

If you look into it in detail, you'll understand what Ruka's illness is, but in terms of a car, even if you step on the gas when you're going up a slope with your luggage loaded, the engine speed doesn't increase and it slides down the slope.
It starts relatively early in life, but about 70% of cases improve or are cured by adolescence or around the age of 20.
If it hasn't increased by Ruka's age, she would need a pacemaker or surgery, but there's no depiction of that.
In fact, she's excited around Kazuya and she was excited at the pool, so if it's true, it's cured.

It's fine if Miyajima wants to wrap up the story by saying she has arrhythmia, but he'll be criticized for being ignorant.
