Eventually someone would see him due to the whole "people can see cursed spirits in mortal danger" thing, at which point Mahito becomes a hole in the ground. They sort of reasoned this ability to see him in Mahito vs Shigaraki, though they had Shigaraki detect Mahito with a combination of his Danger Sense and Search quirks rather than the imminent danger thing.
