She only starts spawning after a specific part of the story around a third of the way in. After that point you'll start getting side quests that feature her and she regularly appears in the slums area. Despite being the hardest enemy in the game she's a complete joke. Unless you purposely refuse to use anything but your finger guns she literally gets stunlocked and the only one that is ever a threat is the Spider's Thread one because she's invincible and one-shots you.
