I think walking across the ocean was specifically mentioned in one of the legion class hall quests, but otherwise the same way they did it last time, raise a few necropolises, teleport in, drop a new plague, etc

Honestly if the scourge did get a new leader they would just need to be smarter than Arthas, in the end he was kind of tried to invade in the dumbest way possible, dropping a tiny number of undead on capital cities, days after his elite super soldiers just rebelled, instead of just teleporting his full force in somewhere remote and spreading out, gameplay will never allow this but the scourge with a smarter leader could be a whole expansion in itself
