Difficulty modes have never been implemented correctly, the original designed difficulty is always obvious
Unlike accessibility for disability, there can't be an objective design method for difficulty modes, leading to every game having its own dumb way of doing it
Difficulty modes will always tend to casualization: think of all the games that force you to unlock harder modes (or even paywall them!). If it's about accessibility, why do the more skilled pay the tax while "easy mode" is prioritized? Hell, why work for hard mode if there's no guarantee it's even well thought out?

Spiritually, difficulty modes muddy intended design by enforcing an expectation for your game to have them, which limits intent and sands it down into designing one "normal mode" to then modulate into other difficulties.
