If you want to get nitpicky, some variant of this may have been planned since 2022. In the earliest build of the game Radhan was a frenzy cultist, and Miquella was trying to destroy the flame of frenzy. It's why the Sleep and Frenzy icons are variants of each other, because they're opposing forces. So Miquella and Radahn having a relationship of sorts was always in the cards.
