Elden ring was well received largely as babby's first souls game, people who played through fromsofts earlier catalogue knew well into the base game's release that it's bloated open world slop. I have roughly 300 hours on two characters, didnt care to finish the game once and never even thought about bothering with the dlc.

I finally mustered up the will to start another run with the idea being 'I will play thru the entire game and give it an honest shot before calling it irredeemable trash', which lasted another 140 hours. If you want to get nitpicky, some variant of this may have been planned since 2022. In the earliest build of the game Radhan was a frenzy cultist, and Miquella was trying to destroy the flame of frenzy. It's why the Sleep and Frenzy icons are variants of each other, because they're opposing forces. So Miquella and Radahn having a relationship of sorts was always in the cards.
