A lot of it stems from the inability to say that you were wrong about something. It's okay and normal to be wrong about things and to admit to it. You wouldn't say you were right about the math problem you messed up? Perhaps it stems from a failure in education and parenting, since many teachers are forced to pass failing students and many parents fail to let their kids learn about how to overcome failure.
